stepped on nail

Hi. My husband stepped on an old, rusty nail during some remodeling around our house today. We didn't worry about it right away, worked for another hour or so, and then he went to the local acute care clinic and got a tetanus shot. However, his foot is now red, swollen and very painful. What should our next course of action be: ER?; keep foot elevated and ice it?; soak in a hot water bath?
