Pain a year after bone spurs on big toe removed

I am a year and 3 mo. post op after bone spurs were removed from my rt. big toe, frayed tendon was repaired and a spacer put in due to no cartilage in joint. I had a reputable podiatrist do it. 3 mo after surgery I started hitting tennis balls and after a couple weeks felt pain in 2nd metatarsal. Turned out to be stress fractured! So back in the boot (critical time for big toe rehab but couldn't move it)!

2 months later I was back at PT and doing ok - began playing tennis 4 months later and a little better. However I am now frustrated cuz I am still swollen and sore at the end of the day and it still hurts throughout the day. I saw an orthopedic doc today and he said he rarely uses spacers and thought it may be too big (looked at X-ray)

but wasn't sure. He didn't have a course of action besides trying the dyna splint? Has anyone had anything similar? I need to be active because I am a personal trainer by trade and besides I love tennis and skiing (which I didn't do last year). All advice is welcome! Thx
