Ongoing arch and metatarsal pain

Hello - I've been dealing with a pain in the arches of my feet for about a year and a half now. Now my 4th metatarsal area hurts whenever I place pressure in that area (so, walking).



A while back, I walked many miles and eventually I started feeling a cramping feeling in the inner mid-arch of my left foot. I continued walking, and the pain started to become enough of an issue that I was limping and favoring my right foot.



it got to a point where I could hardly use that left foot. Eventually the issue started occuring in my right foot, as well. So I had this same sharp debilitating pain in the arches of both my feet -- but only after I walk around a quarter of a mile (or more; the more I walk, the more it hurts until I literally cannot walk anymore). I have used orthotics, physical therapy, MELT method, xrays, MRIs, and found nothing. I would say the only areas of my feet where I generally don't feel any soreness, cramping, or shooting pains are my heel and toes. Now my 4th metatarsal in my left foot has a shooting pain feeling whenever I walk. The pain hurts a little less if I'm barefoot. It does not hurt in the morning, and so I doubt it's plantar fasciitis (specialists have said as much)



The podiatrist and ortho specialist both said it's probably a footwear, biomechanics issue, but I'm not sure anymore. Why wouldn't the custom orthotics have helped?



So anyway, here I am. 1.5 years after this stupid overuse injury, and I feel like I'm nowhere closer to recovery than I was when it first happened.



Does anyone have any advice for me? This issue has taken a physical and emotional toll on me. As I said, I'm pretty desperate, so any suggestions you all might have would be awesome.
